To receive our outpatient mental health services, you will need to complete a biopsychosocial assessment. Assessments are offered via telehealth Monday-Sat between the hours of 9am-3pm. A psychiatric provider will be accommodating you in the biopsychosocial assessment.
* Please note – A biopsychosocial assessment is only to confirm admission into our practice. It is not a substitute for receiving medication right away.
What to submit before scheduling your Biopsychosocial Assessment:
- Proof of Identification and Health Insurance.
- A list of all current medications.
- A copy of the referral, if applicable.
- Completed new patient paperwork.
